Hum, this will not let me reply to your response. But it looks like you did not read my issues. I did not have a problem with filling out the form. I had a problem with you not emailing me that there was an issue with my application. When I checked to see the status of my application that is when I knew there was an issue. I also was never notified by email when my application was accepted.
