They also sent me a credit card authorization form and stated the following: "Please fill out the attached form and return it to us, along with a copy of the cardholder’s state ID and the front and back of the credit card being used to place the order." I have never had an online company ask for a photocopy of my STATE ID!!! I will never use Cost Central again."
