Then logged in to my vision insurance website and printed out the out of network claim form. Filled out the form and mailed it in with my Gaffos invoice and my PayPal receipt. Easy as 123! Gaffos shipping is super fast too. I selected the free 1st class and got my frames in 4 days. Their frame prices were way cheaper than my optometrist so I'll definitely get my frames here from now on "
