"I used Mixbook at mixbook.com for one of my family's photobooks. I started out with one of the Natural themes, but since I changed almost all the backgrounds, I wouldn't do that again--maybe starting with blank might be best. And right at the end I figured out that I don't actually need to choose a layout for each page at all--I can just drag as many photos as I want onto the page and move them around and resize them; also text boxes. One can change all the photos similarly with layouts, anyway. This album is pretty much as good as one I ordered from iPhoto, and much cheaper (though they are different sizes and lengths, too). For future books: I will probably use Mixbook. My ideal would be the flexibility of Mixbook, but with an option of automatically including captions (from titles, or descriptions, or both).

More recently for another year's album, I went with Mixbook again. This time I used the Vintage Year in Review theme, but eventually created a couple of page designs and layouts that I liked (vintage backgrounds and 7- to 9-photo layouts), which I kept copying (before adding photos) for additional pages. Still doesn't import text--and I have to drag and drop from the iPhoto album to a file on my computer, from which to export them into Mixbook. But it still has the flexibility to do many photos per page and allows me to adjust their placement, size, zoom, and so on, and I add whatever text I want (often copying and pasting my captions from iPhoto). It isn't that cheap by the time I've added all the pages necessary for the hundreds of photos, but it's cheaper than many of the alternatives.

The ability to import captions directly into a photo book seems to be the hardest thing to find, as well as the greatest time-saver. It would save me having to cut and paste each caption from iPhoto. Metadata is important--what good are the photos if you (or your grandchildren) don't know who the people are or where the photo was taken? I don't know why this is such a rare capability.
