My only reason for giving four stars is because I am called by both of my first and middle names and those names are on my SSN card and is my legal and called name. The completed application has only my first and last names on it. I mentioned this to Jennifer as soon as I told her my name initially and before any forms were completed. She assured me that the completed forms I would receive would have my full and legal name on them. They do not. This is a great disappointment as I am very sensitive about this and have to correct people all the time about my name. "
