Update: I did NOT, however, expect that the survey I received would automatically cause an account to be created on a 3rd party web site (ResellerRatings.com) and post my survey rating and comment publicly (which I think is the case). There doesn't seem to be a way to get rid of my account and this survey post. A reply email to the survey request from ResellerRatings.com just produced a machine generated response.
This automatically changes my rating to Very Dissatisfied: NOT with the product or price or service from Vitamin Shoppe, but with this privacy encroaching survey site (ResellerRatings.com)."

This gave me some frustrations at the time
Same problem occurred also last year a few times
