On-line chat with a tech person who was also an amateur radio operator was very helpful, educating me on what I was looking at to purchase and some catches to be aware of. Ordering process was not very smooth. I could not ship the item to an alternate address (to keep the gift a surprise) because their "computer wouldn't let them". I was given the option to use PayPal, but did not like the fact that I had to purchase under the alternate address owner's name. So I finally threw in the towel and tried to order outright even though the "surprise" would be lost- but after several tries all night, the order wouldn't go through with no helpful reason given. Tried a few days later and was successful, but then had order held up because their web site didn't estimate shipping high enough. I had to answer an approval email for the shipping which added a bit of delay. This all won't stop me from purchasing from Gigaparts in the future, but I will not look forward to the ordering process. "

If you do not have a way to print the form please let me know and I will USPS mail it to you. I have also attached a copy of your invoice to submit with your rebate.
